This 50 year-old Sonoma County woman had breast augmentation 20 years ago. She noted gradual loss of volume in both breasts with irregularities to their shape. She saw us in consultation in our Santa Rosa Plastic Surgery office. She believed she had saline implants that were now too small, and we made a plan to switch to the new cohesive silicone gel implants. At the time of surgery, she did not have saline implants. She had a "double lumen" implant which was an old implant with part saline and part silicone. All of the saline had disappeared from the implant, accounting for her loss of volume. Through the same areolar incision (using a Keller funnel) new 500 cc silicone gel implants were placed. The pocket was opened to accomodate these larger silicone cohesive implants. The patient is thrilled with her new shape and she loves the feel of the new silicone implants.
